PFS-2 Process
*All families need to complete the PFS2 (Hosting and Family Friend) every time a new
referral is opened.
Options for completing are:
a. emailed to a parent(s) (can send from database)
b. printed and given or mailed to a parent(s)
c. completed with the parent(s) face to face
d. completed over the phone with a parent
*If parent fills out link sent to them it will auto upload to their file on the database. If
you collect any other way, fill it out on their profile under PFS-2.
1. At intake a parent needs to complete the Baseline PFS-2.
a. It is preferred that the parent complete the PFS-2 prior to support fully
starting in order to get the most accurate baseline
2. The Family Coach (FC)/Family Coach Supervisor (FCS) reviews the Baseline PFS-2 to
identify anything needed during Safe Families support
a. The FC/FCS also identifies at this time when the Mid-point PFS-2 target date
will need to be and marks it as a future task to send out
3. A Mid-point PFS-2 will be done at 14 days after start of support unless support is
planned for longer than 30 days. For any arrangements lasting 30 days or longer, a
Mid-point PFS-2 will be sent to the parent halfway through the planned hosting or
family friend arrangement. (ex. If plan is 4 months, do Mid-point PFS-2 at 2 months)
4. The FC/FCS reviews the Mid-point PFS-2 to adjust any supportive services needed
5. 30 days after the closure of the referral the parent completes a Post PFS-2.
*Optional:
- Additional Mid-point PFS-2
- Follow-up PFS-2 collection any time after the Post PFS-2
Tips for presenting to parents
• Use the script on the next page with the parent before collecting
• Explain that this tool will:
o Allow Safe Families to better come alongside them during this time
o Help at an organization level with improving services and increasing potential
for funding to support more families
